Worn Teeth Treated with Porcelain Crowns, Onlays ans Facings

WORN TEETH

Daan had the habit of placing the tip of his tongue between his teeth, causing damage on his front teeth. Besides that, he wanted the golden crowns to be removed. Two of his crowns were screwed on to implants and the screws were clearly visible. He was also grinding and as a result his joints and muscles ached. And finally, he wasn´t very pleased with the looks of his teeth.  As it was going to be extensive treatment, it might as well be good looking too, he thought.

All in all quite some problems and whishes, which we discussed thoroughly before taking the first step: planning the treatment.

The upper and lower arches in full view.

Old golden crowns and inlays, worn teeth and the screws of the crowns on the implants are visible.

We took photos, x-rays and study models of Daan´s teeth.

Cast study models make it possible to study a case and to discuss it with a very important third party: the dental technician. With the models in hand, Daan doesn´t have to be present himself in this part of the treatment.

On the models we can perform simulations of the treatment. Like in this case lifting the bite. A acrylic device was made to check if Daan was able to accept this new situation.

He wore this plate as much as possible day and night.

It wasn´t a problem for Daan to get used to this new bite, on the contrary it felt comfortable.

So now we had a three dimensional copy of Daan´s new bite, the situation in which we would start to make the restorations on the teeth.

The technician can now make a so called wax up, shaping the new form and size of the teeth as they are going to be.

This wax up is easy to copy into the mouth with an acrylic to get a better idea of the aesthetical part of the treatment.

Now we can start preparing the teeth, remove the old crowns etc.

In this case very little was taken off the teeth. While the technician fabricates the new porcelain restorations Daan was wearing provisionals that were more or less copies of the teeth from the wax up.

This gives both dentist and patient the chance of evaluating the bite, the function and the aesthetics once again.

One by one the restorations are placed, very carefully.

The bonding procedure is very critical and once the facings are placed they will not come off.

Finally some photos of the teeh before and after treatment:
